Delete Your Account

Request deletion in the app

1. Sign in to the Quad Therapy app.
2. Open your account settings.
3. Select Delete Account, then confirm Request Deletion.

Request deletion without the app

Email us from the address associated with your Quad Therapy account. Use the subject “Quad Therapy account deletion request” and include your full name and account role. Do not include medical, payment, or other sensitive information in the email.

What happens to your data

After verifying the request, Quad Therapy will delete or de-identify account data that is no longer needed to provide services. This may include profile information, device and notification settings, and optional content associated with the account.

Certain healthcare, billing, transaction, security, fraud-prevention, and audit records may be retained when required by law or for legitimate compliance obligations. Retained information is restricted and kept only for the applicable retention period.
